I understand and agree that I must hold and maintain all licenses required to sell or service the insurance products for which coverage is sought in order to be eligible for coverage under this Errors & Omissions policy.

I understand and acknowledge that this is a claims-made and reported policy. Coverage applies only if the policy is in force at the time the professional service is performed and at the time the claim is first made and reported, with no lapse or gap in coverage.

I understand and acknowledge that coverage, including any applicable prior acts coverage, applies only to the sale and servicing of products designated by the selected coverage option and is subject to all terms, conditions, and exclusions of the policy.

I understand that maintaining continuous Errors & Omissions coverage is my responsibility, and that any lapse in coverage may result in uncovered claims arising from acts, errors, or omissions that occurred prior to or during such lapse.

I understand and acknowledge that I have a duty to notify the Company promptly of any material change in business activities, licensing status, complaints, claims, or regulatory matters occurring during the application process or policy term.
